The District Nursing Team provide skilled nursing care in the patient's own home.
The District Nurses work from Greencroft House Tel No (01461 207697) but can also be contacted by leaving a message with our Receptionist at the Medical Centre.
A large part of the District Nurse's role is to visit and assess the elderly at home and ensure that appropriate services are made available to them by liaising with both Medical and Social Agencies.
The District Nurse is also involved with other age groups, offering services, health education and support in a wide variety of medical and social issues, including pre and post operative care and rehabilitation.

Our Health Visitors are members of the Community Nursing Team working primarily in the patient's home and may be contacted at Greencroft House (01461 201210) and at the Medical Centre (01461 202244)
The Health Visitors can be involved with all age groups offering advice and support on a wide range of medical and social issues. The regularly visit 0 - 5 year olds to advise and inform parents on growth and development, immunisation, accident prevention, behavioural problems, feeding and sleeping. Other aspects of the Health Visitors work are Health Education, visiting and assessing the elderly and liaising with a wide range of Health and Social Care agencies.
